Zur Ablauforganisation des Case Managements gehören diverse Phasen und Schritte, die in ihrer Summe, den Case-Management-Regelkreislauf bilden. Nachdem in der ersten Phase des Regelkreislaufs, der Identifikation bzw. dem Intake, geprüft wurde, ob das vorhandene Klientel für ein individuelles Case Management geeignet ist, folgt daraufhin die zweite Stufe des Re-gelkreislaufs: Das Assessment bzw. die Bedarfserhebung.
